Honeywell Recalls Gas Valves Used in Decorative Fireplaces and Stoves Due To Burn Hazard

Name of Product: Gas Valves Used in Decorative Fireplaces and Stoves

Manufacturer: Honeywell International Inc., of Morristown, N.J.
Hazard: The activation knob on the valve can be left in a position between OFF and PILOT, which can cause the pilot gas to leak and accumulate prior to burner ignition. This can pose the risk of thermal burns to consumers when they light the pilot of the fireplace or stove.
Incidents/Injuries: Honeywell has received one report of gas accumulating and combusting when the consumer lit the fireplace, resulting in a minor injury when the hair in the back of the consumer's hand was singed.
Description and Models: This recall involves gas valves used in liquid-propane-fueled decorative fireplaces and Franklin-type stoves. These stoves and fireplaces have direct pressure relief vents via a spring-loaded door. Fireplaces and stoves with a side or rear vent are not included in this recall. The recalled valves were installed in these fireplaces and stoves.
Sold at: Honeywell sold the gas valves exclusively to original equipment manufacturers for use in fireplaces and stoves from April 2004 through August 2007. Those manufacturers sold fireplaces and stoves at retailers nationwide.

Weil-McLain Recalls Ultra Series Boilers for Carbon Monoxide Hazard

Hazard: These boilers were manufactured for use with natural gas, but could have a blue tag incorrectly indicating to installers that they are
intended for use with LP (propane) gas. If an installer connects one of the boilers to LP gas without installing a propane conversion kit,
carbon monoxide (CO) can build up due to incomplete combustion, posing a risk of CO poisoning.
Incidents/Injuries: Weil-McLain is aware of one incident where two consumers reported CO poisoning when the boiler installed in their home was connected to LP gas.
Description: The recall involves Weil-McLain Ultra 80, Ultra 105, Ultra 155, Ultra 230 and Ultra 310 condensing, high-efficient, gas-fired boilers for space heating. The user's manual and installation manual have "Ultra Gas-Fired Water Boiler" on the cover. The boilers are wrapped in a glossy silver/flat black jacket and either stand on the floor or are wall-mounted. The serial numbers range from CP5071716 through CP5327000. When the panel on the front of the boiler is removed, a bar-coded label with the serial number is located on the lower right hand side of the boiler itself. "Weil-McLain" is written on the front panel of the boilers.

Incidents/Injuries: Triangle Tube has received one report of a flue leak with the recalled Combination Water Heater during installation. No injuries have been reported.

CPSC Announces Vent Pipe Manufacturer Pulls Out of Recall Program -- Other Firms Continue to Honor Recall Agreement
11/7/05 -WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing today that in early 2005 Falcon HC Holding Corp., of Stamford, Conn., withdrew from participation in the recall program to replace Ultravent-brand plastic vent pipes used to vent home heating systems. Because of Falcon's withdrawal, consumers with Ultravent pipes now have to pay the portion the firm had agreed to pay, about 40 percent of the total replacement cost, which amounts to between $230 and $250 per system.
All the other firms participating in the recall program continue to honor their obligations under the program. Falcon's withdrawal does not affect consumers with Plexvent-brand vent pipe. Thus, consumers with Plexvent-brand plastic vent pipes continue to get a cost-free remedy when they replace their vent pipes.
The original recall program was announced on February 24, 1998 with the participation of virtually the entire furnace and boiler industry together with the two manufacturers of high-temperature plastic vent (HTPV) pipes. Its purpose was to replace the estimated 250,000 HTPV pipe systems attached to gas or propane furnaces or boilers in consumers' homes. The HTPV pipes could crack or separate at the joints and leak deadly carbon monoxide (CO). About 93,000 HTPV pipes have been replaced as of October 1, 2005.
When the recall program began in February 1998, Hart and Cooley Inc., the manufacturer of Ultravent pipes, agreed to participate in the recall indefinitely. In December 1999, Hart and Cooley Inc. was purchased by EQ Corp., of Norwalk, Conn., a subsidiary of Equilease Corp., and renamed Falcon HC Holding Corp. The firm currently known as Hart and Cooley Inc., now owned by Tomkins PLC, is not the subject of this press release.
Despite having ample assets to continue funding Falcon HC Holding Corp.'s share of the recall, EQ Corp. has informed the Commission that it is not legally responsible and, therefore, will not do so.
Since the recalled HTPV pipes can leak CO, consumers are urged to replace them immediately. CO is a colorless, odorless gas. Initial symptoms of CO poisoning are similar to the flu, and may include dizziness, fatigue, headache, nausea and irregular breathing. High-level exposure can lead to death.
To determine whether they have HTPV pipe systems subject to the recall, consumers should check the vent pipes attached to their natural gas or propane furnaces or boilers. Vent pipes subject to this recall program can be identified as follows: the vent pipes are plastic; the Plexvent vent pipes are black and the Ultravent vent pipes are gray; and they have the names "Plexvent," "Plexvent II" or "Ultravent" stamped on the vent pipe or printed on stickers placed on pieces used to connect the vent pipes together.
Consumers should also check the location of these vent pipes. For furnaces, only HTPV systems that have vent pipes going through the sidewalls of structures (horizontal systems) are subject to this program. For boilers, all HTPV systems are subject to this program. Other plastic vent pipes, such as white PVC or CPVC, are not involved in this program.
After checking the vent pipes, consumers should call the special toll-free number (800) 758-3688, available between 8 a.m. and 7 p.m. ET, Monday through Friday, to verify that their HTPV pipe systems are subject to this recall program.
Consumers with eligible systems will receive new, professionally installed venting systems free of charge or at a reduced charge, depending on the manufacturer of their HTPV pipes. In addition, furnace owners with HTPV pipes may choose to replace their existing mid-efficiency furnace with a new, high-efficiency furnace manufactured by the same company. Furnace owners with Plexvent pipes will receive a $400 rebate when the work is completed, and furnace owners with Ultravent pipes will receive a $250 rebate when the work is completed.

The recalled valves were manufactured between July 25, 2005 and August 14, 2005 with production date codes 5-31 through and including 5-33, although not all valves with these date codes are affected. The recalled valves were installed on the following water heater brands: American Proline, Bradford White, GE, GSW, Hotpoint, Jetglas, John Wood, Lochinvar, Premier Plus, Powerflex, Rheem, Richmond, Ruud, Vanguard, Whirlpool, and U.S. Craftmaster. The model and serial number can be found on the manufacturer's label on the water heater. Electric water heaters are not included.

Distributor: Tile Perfect, a division of Roanoke Companies Group Inc., of Aurora, Ill.
Hazard: The product's odor is not chemically pungent enough to force consumers to minimize their exposure to the fumes. Consumers overexposed to these fumes can experience respiratory-related illness.
Incidents/Injuries: There have been 88 reports from consumers who have had adverse reactions after using the aerosol product, including 28 confirmed reports of overexposure resulting in respiratory symptoms for which medical attention was sought for coughing, irritation, difficulty breathing, dizziness and disorientation. Thirteen individuals required medical treatment including overnight hospitalization.

Hazard: The burner tube connection to the gas valve can leak gas when the main burner is on. This leaking gas can ignite, causing a minor flare-up, which could cause nearby combustibles to ignite.
Incidents/Injuries: DESA has received one report of a minor flare-up of the fireplace. DESA has received no reports of personal injury or property damage.
Description: The recall includes DESA Heating Products 10,000 Btu Thermostatically Controlled Compact Classic Hearth Fireplaces that burn propane or natural gas. They have the following model names and number: Vanguard (VMH10TPC & VMH10TNC) and FMI (EFS10TPA & EFS10TNA). They have serial numbers between 015200000 and 017400000. The model and serial numbers are on the end of the shipping carton and on the model data tags inside the lower access door of the compact fireplaces.

Incidents/Injuries: The firm has received three reports of malfunctioning lower guards, including one amputation.
Description: The recall circular saws include the BOSCH CS10, CS20 and CS20-XC models, which can be identified on the product nameplate mounted on the top of the motor housing. The recalled models do not have an asterisk (*) in the serial number box. The circular saw uses a 7¼-inch blade and has a blue handle and motor enclosure, and a metal guarding system. The name "BOSCH" is printed on the blade guard.
Sold At: Home Depot, Lowe's and Menards nationwide, as well as independent hardware retailers and industrial distributors, from February 2004 through April 2005 for between $120 and $150.
Remedy: Consumer should stop using the circular saws immediately and contact Robert Bosch Tool Corp. to receive a repair kit. The kit includes hardware and instructions for installation.

Hearth & Home Technologies Issues Safety Alert Regarding Heat & Glo Brand GEM 36 and GEM 42 Gas Fireplaces
Hearth & Home Technologies wants to inform consumers of possible safety risks associated with the continued use of 7,815 Heat & Glo(TM) brand GEM 36 and GEM 42 gas fireplaces sold since July 2002. The fireplaces can, under certain circumstances, accumulate gas prior to burner ignition, causing the glass window to shatter and presenting the risk of burns or cuts from broken glass.
"We are asking customers who own one of these products to turn off the gas flow to the fireplace and stop using it until we can send someone to their home and correct the problem at no expense to them," said Brad Determan, president of Hearth & Home Technologies.
Determan explained that company representatives are notifying customers as quickly as possible, either directly and/or through dealers and distributors who sold the fireplaces. Heat & Glo gas fireplace owners can determine if they own a GEM 36 or GEM 42 by checking the rating plate in the bottom of the unit located on the base pan in front of the gas control or by calling Heat & Glo Customer Care at 1-800-215-5152. CPSC Signs Cooperative Safety Agreement with the European Commission at International Safety Conference
WASHINGTON , D.C. - The U.S. Consumer Product Safety Commission (CPSC) and the Directorate-General Health and Consumer Protection of the European Commission (EC) entered into an information sharing agreement at a ceremony hosted by the International Consumer Product Health and Safety Organization (ICPHSO) during its annual meeting and training symposium in Orlando , Fla.
CPSC Chairman Hal Stratton and Director General Robert Madelin from the EC signed letters of commitment to implement mutually agreed guidelines to strengthen communication between both entities, and to improve consumer health and safety protection in the United States and the European Union. The agreement also addresses the need for enhanced regulatory cooperation on consumer product safety issues highlighted as an important element in the Roadmap for U.S.-EU Regulatory Cooperation announced at a June 2004 US/EU Summit.
"The exchange of scientific, technical, and regulatory information will benefit consumers on both continents," said Stratton.
The cooperative safety agreement with the EC is one of several that CPSC has negotiated in recent months with representatives of foreign governments. CPSC also has formal working relationships with Canadian, Chinese, Taiwanese and Costa Rican authorities. CPSC is negotiating Memoranda of Understanding with Mexico , India and Chile . The goal of these agreements is to heighten awareness of the global aspect of consumer product health and safety issues and to foster the exchange of information that will help reduce the incidence of death and injuries associated with consumer products.

The recall involves only models manufactured from September 2007 through October 2007. The model number and the manufacturing date code are located on the left side rail of the ladders. Ladders with two green dots near the bottom of the left side base rail have already been inspected or repaired and are not included in the recall.
